Clinical pharmacists are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out structured medication reviews and managing patients with long term conditions.
They can also respond to acute medical issues assessing a patient to determine the proper medication therapy to issue. They can explain treatment regimens and educate patients on how to take these safely.
Pharmacists have an active role in interpreting clinic letters received from secondary care and action any medication changes giving advice and education to these patients.
Pharmacists may also carry out safety audits and you may get a call from them if they feel you are on a medicine which needs further information
