A team of Clinical Pharmacists work across our five practices consulting with and treating patients. This includes helping patients to manage long-term conditions, supporting those on multiple medicines, carrying out structured medication reviews and enabling better access to health checks.
The role is pivotal to improving the quality of care and ensuring patient safety. Having Clinical Pharmacists in GP practices also helps GPs manage the demands on their time, for example, diagnosing and treating patients with more complex conditions.
